Reports are 'Woolf' compliant and compatible with
both the Civil Procedure Rules 1999, it's amendments and Guidance from the Expert Witness Institute
2001.
Clients
are offered an appointment for examination within one week of
receiving written instruction.
The report is forwarded within
14 days of examination (unless
delayed by need for medical records).
We ask for settlement
of invoice within three months of
date of issue unless by special arrangement.
Standard reports are
formatted over five pages and include the following details :-
* Client
factors including name, address, date of birth, date of accident, date and place
of examination,
date of report, instructing agent/solicitor
* Summary of injuries
* Source of instruction
* Review of any necessary medical records
* Qualifications
of the reporting
Author
* Description of incident giving rise to injury
* Chronological progression of physical & psychological
symptoms including treatment
* Current symptoms & present situation
* Effect on lifestyle including work, leisure, home & psychological stress
* Clinical findings on medical examination
* Conclusion including opinion, prognosis & recommendations
* Declaration & Statement of Truth
