I have been asked to prepare a medical report
concerning personal injury. My instructions have been received from

expressed in the report are independent of any suggestion by others, including my
instructing agent, and the report is written as if it were being sought by the Judge of a
Court of Law.

This report has been prepared following interview
and examination of the claimant on **** and after sight of the
copied medical record submitted by **** including hospital details (A &
E No ****) and medical practitioner notes.
I
will notify my instructing agent immediately and in writing should I have reason to modify

QUALIFICATIONS
(BSc MSc MBBS MRCGP DRCOG)
I qualified from St Thomas' Hospital Medical School,
London in 1979. I have seventeen years continuous service as a Principal in General
Medical Practice, twelve years as Forensic Medical Examiner and eight years as Clinical
Tutor to St George’s Hospital Medical School, all posts that I currently hold.
I hold previous specialist experience in both Rheumatology and Orthopaedic
Surgery and have prepared personal injury reports over the past eleven years. I hold
full registration with the General Medical Council and I am an elected member of the
Expert Witness Institute.
